Lincolnshire Show: Carer Application

Guests who require a Carer for the 2024 Lincolnshire Show are entitled to one Carer/Personal Assistant ticket at no extra charge.

Use this form to apply for a free ticket. A member of the Showground Team needs to verify each form and then instruct SeeTickets to issues a free ticket.

Please note this form will ask you for a SeeTicket reference. Please purchase any non-carer tickets from SeeTickets before completing this form. If you are a Member of the Society, and receive tickets under your membership, please state your membership number below.

Applications open until 10am, Tuesday 11th June 2024. Applications after this time will not be considered.

Should you wish not to complete this form or pass the deadline, on the day carer tickets will be available from any of the pay booths. You will still need to provide the below evidence and purchase all none-career tickets on the day.

Name of carer The name the tickets will be issued under.

Name of person who requires a carer/personal assistant This name much match the supporting document.

Email This must match SeeTickets/Membership and is where your additional ticket will be sent to.

Are you a member of the LAS?

See Ticket reference number or Membership ID This will be sent via email from SeeTickets after purchase of your non-carer tickets.

PLEASE NOTE: This form is not a ticket. Each application has to be verified by a member of the Lincolnshire Showground Team who will check your details. Once confirmed you will receive one free ticket, via email, from See Tickets. It is your responsibility to make sure you receive this ticket with 5 working days of submitting this form.

One of these is required to apply

Entitlement to Disability Living Allowance for children under 16 or DLA/Personal Independent Payments (PIP) for those aged 16-64, either in the form of a letter stating that the benefit has been awarded, or the actual Allowance book
Attendance Allowance or Carer's Allowance letter of award
Incapacity Benefit books, or a letter notifying the recipient that the benefit has been awarded Incapacity Benefit or Employment and Support Allowance (ESA)
A valid Blue Badge
In the case of visual impairment, a registration card known as the BD8 or a Certificate of Visual Impairment (CVI)
A local authority registration document which is local to the attraction where it is being submitted
A recognised Assistance Dog ID Card
Credibility’s Access Card with +1 symbol
A personal letter from GP, community nurse or social worker
War pensions mobility allowance
